What can I expect as a new patient?
- We ask that you arrive for your first appointment 30 minutes early so that you can fill out a few medical history forms. These forms can be downloaded from the patient portal and brought with you. If you have any questions, or need assistance, please ask us, we’re happy to help.
- If you are on any medications, please bring them with you.
- We encourage you to keep a log of your blood pressure, sugar readings, miles walked, etc., and to bring your log with you.
- If you have health insurance, we will make a copy of your insurance card so please bring that with you.
How can I schedule an appointment?
- Call 210-256-1539 and let us know when you would like to come in.
- Existing patients visit the patient portal at any time to make or reschedule an appointment.
What is the Patient Portal?
After signing up, creating your username and password, you can make appointments, send messages, pay your bill, download forms as well as many other features. Visit your Patient Portal
What insurance do you accept?
